What permissions should I get if I want to quote a musical score fragment in a book?

It is extremely difficult and expensive. You would need to pay a licensing fee to ASCAP or BMI - whichever the author of the song is registered with. You would have to apply and they would have to know the exact usage and then they may or may not issue you a license. Most publishers and editors will take song lyrics out of a manuscript to avoid paying the fees which are a LOT. If you choose a song in public domain, you have no problem, but otherwise it is just not advisable. You can, however, use a song title which cannot be copyrighted. So the theory is that if you mention a song title, readers will automatically start thinking of the lyrics in their mind. That is how you get around using the actual lyrics. So if you said, "John got on the elevator whistling the Rolling Stones' tune Satisfaction", you have made your point without using the actual lyrics. Your readers will "get it" and you just saved you and your publisher a bundle on licensing fees and a literary attorney to file for them.

I had a situation like this come up. And believe it or not, until the most recent change in copyright laws, the song would have been in public domain. However the laws changed and now the song misses out on public domain by less than ONE year. I had to take it out.
